Trade/Service Mark
Mark shall mean a sign in the form of a picture, name, word, letters, numerals, composition of colours, or a combination of said elements, having distinguishing features and used in the activities of trade in goods or services.

REQUIREMENTS FOR TRADEMARK APPLICATION* Complete data of the Applicant and the Mark.
* Power of Attorney, simply signed by the Applicant* Declaration of Entitlement of Mark, simply signed by the Applicant* 30 prints of the mark. In min size of 2x2 cm and max size of 9x9 cm
* Certified copy of Priority Document (if priority is claimed). Documents made out in any other language than the English, translation into English by sworn translator is compulsory.

REQUIREMENTS FOR TRADEMARK RENEWAL
* Power of Attorney, simply signed by the Applicant
* Declaration of Entitlement of Mark, simply signed by the Applicant
* Copy of the previous certificate of trademark registration
* 30 prints of the mark. In min size of 2x2 cm and max size of 9x9 cm

REQUIREMENTS FOR RECORDAL OF CHANGE OF NAME * Power of Attorney, simply signed by the Applicant* Original or certified true copy of the document of change of name.
(If the document of Change of Name is made out in any language other than English, translation by a sworn translator is compulsory)

REQUIREMENTS FOR RECORDAL OF CHANGE OF ADDRESS * Power of Attorney, simply signed by the ApplicantREQUIREMENTS FOR RECORDAL OF ASSIGNMENT* Power of Attorney and Statement of Use, simply signed by the Assignee
* Declaration of Ownership, simply signed by the Assignor.* Deed of Assignment, notarized and attested by the Indonesian Embassy/Consulate.* (Deed made out in any language other than the English has to be translated by a sworn* translator).

VALIDITY A trade/service mark registration will be valid for ten (10) years computed retroactively from its filing date. It is renewable for the same term subject to the renewal procedure.
